What the DDP Offers

The program offers no cost services to qualified individuals with a developmental disability that occurred before age 18. Services focus on daily support, skill building, and long-term quality of life.

Designed to serve individuals with developmental disabilities across all ages, the DDP helps people lead full, enriched lives while staying active in their communities. Through a range of services and providers including self-directed care options, families can find the tools they need to thrive.

Medicaid 0208 Waiver Services

This Home and Community Based Services waiver offers comprehensive long-term support for individuals with significant developmental delays.

Additional DDP Waiver Benefits

Express Healthcare also offers Individual Goods and Services (IGS)

This means we can front the cost for approved items in your cost plan or reimburse families for purchases that are approved. This helps ease the financial burden of getting the necessary goods or services your loved one needs.

Applied Behavior Analysis (ABA)

ABA therapy supports growth in communication, social skills, and behavior through evidence based strategies and positive reinforcement. ABA can be especially helpful for • Autism Spectrum Disorder (ASD) • Intellectual or developmental disabilities • Children and young adults with social or learning challenges.

Targeted Case Management (TCM)

Available to individuals 16 and older who are receiving or eligible for 0208 Waiver services. Designated Case Managers are here to walk alongside you coordinating care, connecting you to services, and helping tailor a plan that fits your family’s goals.

Evaluation

The eligibility process may involve several different people in your home to make evaluations and ask questions.

Documents

 You may also need to submit doctors’ reports and evaluations, hospital records, school district evaluations, or other documentation to determine if the disability meets the requirements set by the state for eligibility.
