The hours recorded accurately reflect the time I worked and tasks completed.
I have not worked beyond my client’s authorized hours. I understand unauthorized time will not be compensated and may be adjusted to meet state regulations I was given by Express.

Late timesheets may be processed in the following pay period.
Incomplete or illegible timesheets will be returned unpaid for correction.
I was not injured during my shift. If I was injured and do not require emergency care, I will call 877-576-8148 (Option 1) and provide location number 1078.
I understand scanned timesheets are preferred and photos may not be accepted.
